Job description
ESSD Team Rehabilitation Assistant

Band 3 Rehabilitation Assistant based in Eden – 15 hours per week. This role supports stroke survivors in the community following discharge from hospital, providing high‑quality rehabilitation across the patient pathway and working flexibly within community and ward settings.

Responsibilities
  • Work as a lone worker with occasional supervision from qualified staff.
  • Carry out simple assessments, implement and evaluate rehabilitation interventions and the use of routine equipment.
  • Assist patients with daily tasks using a rehabilitation approach (personal care, washing, dressing, bathing, etc.).
  • Maintain accurate written and electronic records and complete daily administrative duties.
  • Collaborate with multidisciplinary teams to support patients across home, community, and acute hospital settings.
  • Participate in service development, contributing to the modification of team interventions.
  • Provide flexible working, including 7‑day coverage and potential rotation between community and ward settings.
Qualifications & Experience
  • Care certificate or willingness to undertake one.
  • Experience working with stroke patients or a comparable rehabilitation setting is desirable.
  • Ability to work independently, maintaining professional judgement and adherence to safe practice.
  • Strong communication skills and a commitment to patient‑centred care.
  • Good computer literacy for record‑keeping and reporting.
