We are currently recruiting an experienced and motivated Care Coordinator to join our Hereford branch. This is an exciting opportunity to take part in the continued growth of our service, ensuring quality, compliance, and operational excellence.

Role Overview

The Care Coordinator plays a pivotal role in the smooth running of day-to-day care delivery. The successful applicant will organise staff rotas, manage client care plans, and ensure a high standard of service that meets CQC requirements.

Key Responsibilities
  • Accept care packages and allocate care workers appropriately based on skills and client needs.
  • Produce and manage carer rotas efficiently to ensure continuity of service.
  • Ensure service user care plans and staff files are complete and accurate.
  • Monitor staff attendance and punctuality via electronic monitoring systems.
  • Arrange care reviews and risk assessments with the field supervisor.
  • Conduct service user calls and home visits to ensure quality assurance.
  • Liaise with clients, families, local authorities, and other stakeholders professionally.
  • Support recruitment, onboarding, and supervision of care staff.
  • Represent the company during meetings with officials and commissioning bodies.
  • Ensure compliance with CQC regulations and company policies.
  • Self‑motivated and proactive approach to work.
  • Highly responsible with excellent communication skills.
  • Strong problem‑solving ability and critical thinking.
  • Excellent organisational skills and attention to detail.
  • Reliable, trustworthy, and professional at all times.

  • Level 2 Diploma in Health and Social Care (QCF) or equivalent qualification.
  • Strong understanding of CQC compliance and care standards.
  • Excellent IT and administrative skills, including use of care management systems.
  • Full UK driving licence (mandatory).
  • Experience using Care Planner or similar electronic scheduling software.
  • Previous hands‑on experience as a care worker.
