  • To work in partnership with service users, families, and carers to achieve identified goals and to support recovery and reintegration by adopting a fully holistic approach to needs.
  • To accurately record clinical information into our systems including risk assessments, care plans, care act assessments, crisis and contingency plans.
  • To work within a fully integrated and jointly managed Community Mental Health Team alongside Local Authority social care staff and managers and to liaise and feedback on all aspects of service user care.
  • To assess any social care needs of your caseload and look to source and arrange these in line with the operational policies.
  • To mentor and supervise both preceptee nurses and student nurses, enhancing their placement experience and supporting their learning and development.

  • We Care – We act with respect and empathy, and always value difference.
  • We Listen – We understand people’s views and respond to their individual needs.
