Community Healthcare Assistant

An opportunity has arisen for a Band3 Healthcare Assistant to join our busy neighbourhood team. You need to be a motivated, flexible, resourceful, caring and compassionate individual. You will be supported by a highly skilled multi-disciplinary team to deliver high quality care whilst supporting patients with a variety of conditions and needs within their own homes.

We are looking for a candidate with excellent communication skills as you will work closely with other professionals, outside agencies and patient families. It is essential that you can meet the travel requirements of the role.

This is a part time post working 30 hours per week. Please note the Trust will not routinely provide sponsorship to work for this role. In addition to meeting Home Office salary threshold you must have significant experience to be offered top of the pay band and work a minimum of 35.5 hours per week.

Main duties of the job
  • Engage with individual patients to build rapport and enable them to achieve maximum independence with activities of daily living, e.g. washing, dressing, grooming, meal preparation, eating and drinking, and functional transfers (toilet, bed, chair).
  • Carry out remedial activities to develop physical, perceptual, emotional and cognitive function.
  • Follow appropriate training and competency assessment, and carry out specific tasks delegated by a qualified nurse or therapist such as wound care, dressings, insulin administration and medication administration in line with Trust policy.
  • Fit and demonstrate the safe use of equipment as directed by the qualified clinician.
  • Work with other professionals within the multi-disciplinary team.
  • Liaise with external parties such as social workers and GPs.

Job responsibilities

Provide a high standard of outcome-oriented care and support, following a care / rehabilitation / recovery plan, under the supervision and direction of qualified clinical staff for patients in their own homes.

Knowledge

Qualifications
  • Level 3 qualification in Care or equivalent, or Level 2 qualification in Care and expected to undertake the relevant Level 3 qualification on appointment.
  • Will be expected to meet the Care Certificate standards within 12 weeks of commencing employment with the Trust.
  • Submit original certificates to verify qualifications at the point of offer.
  • Intermediate Skills Programme.
Experience
  • Experience of working with older adults in a health/social care setting.
