An exciting opportunity has arisen for enthusiastic, ambitious and forward‑thinking individuals to join the Central Primary Care team supporting clusters across Conwy and Denbighshire. The position holder will have a particular focus on Conwy East. Primary and Community Care is an exciting place to work right now. Welsh Government are investing in clusters and localities across Wales to serve the local population in a whole system approach.
The successful candidates will work alongside cluster leads and locality members to develop the roles and functions of localities in line with Welsh Government and Health Boards locality developments. The co‑ordinator role will be responsible for providing support and facilitation to clusters and localities, supporting the population needs through effective and robust planning and service delivery, whilst leading development at pace.
Post holders will also ensure primary care and wider locality services are developed and deployed effectively as a critical part of a strong primary and community model of service delivery. If you think you have the skills and ability to take the localities to the next level of health and social care and ambition to shape the face of the whole health economy across Conwy and the ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
