Clinical Pharmacist – Primary Care Network

Location: Taunton
Hours: Part-Time or Full-Time
Salary: Up to £60,000 per annum
Working Pattern: Hybrid working possible

Job Summary

A Primary Care Network in Taunton is seeking an experienced and motivated Clinical Pharmacist to join its multidisciplinary team, based within a GP practice setting. This role offers the opportunity to play a key part in delivering high-quality medicines optimisation, structured medication reviews and long-term condition management.

The post-holder will work closely with GPs, pharmacy technicians and wider practice teams to improve prescribing safety, reduce workload pressures and deliver patient-centred care. As part of a PCN recognised as a Learning Organisation, the role offers strong support for professional development, education and leadership.

Main Duties

  • Conduct structured medication reviews, particularly for patients with polypharmacy, complex needs and long-term conditions
  • Support long-term condition clinics (e.g. asthma, COPD) through face-to-face and remote consultations
  • Manage acute and repeat prescriptions within scope of practice (including independent prescribing where qualified)
  • Undertake medicines reconciliation following hospital discharge to ensure safe transitions of care
  • Support care homes and complete domiciliary medication reviews where required
  • Respond to MHRA alerts, prescribing queries and formulary updates
  • Lead and contribute to quality improvement, audit and safer prescribing initiatives
  • Provide medicines-related advice and education to practice and PCN staff
  • Work collaboratively with GPs, pharmacy technicians, community and secondary care teams

Person Specification (Summary)

Essential:

  • Registered with the General Pharmaceutical Council (GPhC)
  • MPharm (or equivalent)
  • Strong knowledge of primary care prescribing and medicines optimisation
  • Excellent communication, interpersonal and organisational skills
  • Ability to work independently and as part of a multidisciplinary team
  • Good IT skills and ability to analyse clinical data

Desirable:

  • Independent Prescribing qualification
  • Experience working in general practice or a PCN
  • Membership of the Royal Pharmaceutical Society
  • Completion of, or working towards, a Primary Care Pharmacy Education Pathway
