The Care Team Clinical Lead (CTCL) is a crucial role in promoting and maintaining a positive overall client and family experience and ensuring safe and effective care in compliance with VHA’s policies, Ontario legislation, and standard of practice. The primary role of the CTCL is to conduct in-home client visits to assess, develop, and monitor Client Care Plans; provide clinical support, education, and training to personal support workers for delegated acts as well as special functions. Follows up with PSWs to ensure their skills and competencies are up to date and monitor clinical documentation; liaises with Ontario Health atHome.
