Qualifications

  • Relevant qualification in childcare, education, health, or social care (NVQ 3 or equivalent/higher).

  • Understanding of trauma and its impact on young people.
  • Experience in assessing needs and managing risks in complex situations.
  • Proactive attitude towards helping care leavers achieve independence.

Responsibilities

  • Provide tailored support and advice to care leavers aged 16–25.
  • Build positive relationships and encourage participation in decision-making.
  • Advocate and explain complex information in accessible ways.
  • Assess individual needs, identify risks, and set realistic goals.
  • Manage a caseload and complete assessments within required timescales.
  • Advise on financial entitlements and ensure leavers receive support.
  • Coordinate services with partner agencies.
  • Support access to training, education, and employment opportunities.
