Job summary

Advanced Clinical Practitioner Lead - Countywide Community Services

37.5 hours per week, to suit service needs, permanent.

Working as part of the senior team within community services, you will be responsible for the development, clinical leadership and operational management of a cohort of advanced nurse and therapy practitioners, supporting them to develop competencies within all pillars of advancing practice and also clinical leadership for enhanced clinical practitioners within MIUs,

Due to the nature of the post the applicant must be able to travel independently.

Main duties of the job

You will be the figurehead for developing advanced clinical practice within community hospitals and MIUs, whilst aligning that work to wider community services in the future. You will need to have the ability and experience to work within a dynamic and challenging environment and you will play a pivotal leadership role supporting the wider workforce to enhance clinical skills and competencies to support patients living with longterm conditions and frailty.

You will have a track record of success at a senior level, and have the clinical skills and abilities to develop credible trusting relationships with clinical colleagues within the Trust and the wider health care economy.

Enthusiastic and passionate about patient experience and clinical outcomes, you will have an understanding of the national agenda and be able to translate this with commissioning and alliance partners to lead clinical delivery. You will be able to work alongside directors, GPs and commissioning and social care colleagues to shape future planning of community services.
