About the role

We are seeking a proactive, skilled, and efficient Social Worker to join our Learning Disability, Autism, and Hearing & Vision service. This is a crucial short-term role focused on supporting our commitment to delivering high-quality, person-centred practice across Adult Social Care.

We are looking for an experienced Social Worker who can confidently and autonomously manage a defined workload, with a particular focus on completing annual reviews. The primary purpose of this role is to work efficiently through pending reviews, ensuring they are completed thoroughly, succinctly, and in line with statutory requirements, helping us reduce waiting times and improve the experience of the people we support.

Key Responsibilities
  • Complete Care Act annual reviews for adults with learning disabilities, autism, and sensory needs.
  • Ensure all reviews are strengths-based, proportionate, and outcome-focused.
  • Work collaboratively with individuals, carers, families, and partner agencies.
  • Maintain accurate, high-quality case records and documentation.
  • Apply professional judgment, safeguarding principles, and relevant legislation in all practice.
  • Contribute to efficient service delivery through effective time management and communication.
