Job summary

A health service provider in Chester is seeking a proactive Case Manager to join their Acquired Brain Injury service. Qualifications

• Experience working with individuals with an acquired brain injury.
• Minimum 5 years post-graduate experience, including 2 years in rehabilitation.
• Demonstrate learning at Master’s level or equivalent.

Responsibilities

• Coordination of interdisciplinary collaboration for clients.
• Manage initial screenings, risk assessments, and interventions.
• Travel around the Wirral for client visits.
