Job summary

A local government authority is seeking a dedicated Social Worker for the Learning Disabilities Transitions Project. The role focuses on completing Care Act assessments for young adults transitioning from children's services. Candidates must have a Social Work degree and be registered with Social Work England. You will work with families, provide high-quality support, and collaborate with a multi-disciplinary team. This position offers an hourly pay of up to £32.

Qualifications

  • Degree in Social Work and current registration with Social Work England.
  • Proven experience assessing needs under statutory responsibilities.
  • Knowledge of the impact of Learning Disabilities, Autism, and ADHD.

Responsibilities

  • Complete Care Act assessments for young people in transition.
  • Utilize specialist knowledge to provide high-quality support.
  • Carry out statutory responsibilities under relevant acts.
