About the role

Brighton and Hove is a vibrant and exciting place to live and work and an opportunity has arisen to join the Hospital system social work team.

Both the acute hospital and community arms of our wider team have seen their roles significantly evolve in recent years as we have been at the forefront of changes with the hospital’s Discharge to Assess model. This has meant our team work with people in the acute setting as well as those in community settings after a period of time in hospital. This can be either in health beds whilst the person is benefitting from rehab or in the person’s home. We also work with those who need support to prevent a hospital admission including playing a role at the front door of the hospital by working with our health colleagues in the Emergency Department.

We are a friendly and supportive team working in a busy environment; although working under pressure we maintain high professional standards of practice and are committed to ongoing professional development.
