Norfolk Swift Response Reablement Liaison Officer (Nights) | 9521 | Permanent Contract | 20 hours per week | £14,271 to £15,211 per annum | (Grade E)

Hybrid working (County Hall, Norwich)

There is a market supplement of 10% to the base salary to all staff who are employed in night roles. This will be paid three times a year.

Working Pattern

This is a hybrid role, with shifts split between home and office. Shifts are 9:45pm to 6:15am, including weekends and bank holidays on a two-week rota. Week 1: no shifts. Week 2: Wednesday, Thursday, Friday, Saturday, Sunday.

Are you ready to play a vital role in a fast-paced, responsive social care service? Norfolk Swift Response is looking for Reablement Liaison Officers to join our dedicated team providing essential coordination and support that helps vulnerable service users.

What You’ll Be Doing

As a Reablement Liaison Officer, you’ll be the first point of contact for referrals and queries, working closely with GPs, community health teams, social services, service users, and their families. You’ll be part of a supportive and friendly team, and your responsibilities will include:

  • Taking referrals via phone and our social care system (Liquid Logic)
  • Working alongside the East of England Ambulance Service to respond to incoming referrals for Swift and Night Owl services
  • Triaging incoming information against service eligibility criteria, escalating to a manager where unclear, and making timely decisions on referrals
  • Planning and coordinating staff logistics, ensuring the right teams are sent to the right calls, making effective use of staffing resources
  • Deploying support workers to respond to unplanned care needs such as falls, personal care, or welfare checks
  • Providing real-time support to our support workers in the field, answering queries and offering a calm, reassuring voice
  • Supporting referrals into Norfolk First Support
