Job Description
Overview of Role / Essential Job Functions
Provides basic nursing care to residents within the scope of the nursing assistant responsibilities and performs basic nursing procedures under the direction of the licensed nurse supervisor.

Resident Rights
- Knows and respects patient rights.
- Ensures all protected health information is kept confidential.
- Reports all complaints made by patients to supervisor.
- Reports all allegations of resident abuse, neglect and/or misappropriation of patient properly.
Safety and Sanitation
- Follows established safety policies and procedures.
- Observes safety needs of patient as indicated in care plan.
- Wears and/or uses safety equipment and supplies when indicated and properly trained to use.
- Demonstrates job-specific knowledge of fire and disaster preparedness during drills or actual situations.
Staff Development
- Attends and participates in scheduled in-service training, educational classes and meetings, including safety committee.
- Participates in QAA process.
Qualifications
Responsibilities
Personal Nursing Care Responsibilities
- Participates in and receives nursing report upon reporting for duty.
- Assists residents with resident care including bathing, grooming, hygiene, and placement of adaptive equipment
- Explains procedures to residents before administering
- Keeps residents dry (changes gown, clothing, linen, etc., when it becomes wet or soiled)
- Changes bed linens and keep linens tight to avoid wrinkles from forming under residents
- Make beds (occupied and unoccupied)
- Handles linens according to infection control guidelines
- Assists residents with bowel and bladder functions
- Assists residents in preparation for and transporting to appointments, activities and social programs
- Assists with lifting, positioning and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc., in keeping with specific resident safety needs
- Assists residents in ambulation with or without self-help devices as instructed
- Performs restorative procedures as instructed
- Accurately measures and reports heights and weights
- Accurately measures and records temperatures, pulse, respirations and blood pressures (TPRs and B/P) as instructed
- Empties urinary drainage bags and reports amount in record as indicated; reports any irregularities to nurse
- Ensures call lights are within reach of residents and answers call lights promptly
- Ensures that residents who are unable to call for help are checked on frequently
- Ensures that residents’ personal care needs are being met in accordance with residents’ wishes
- Reports all changes in residents’ condition to supervisor as soon as practical
- Completes appropriate charge and inventory slips and monitors supply inventory daily to see what is needed; cleans instruments and equipment and returns to proper area after use
- Straightens dresser tops and stands
- Maintains cleanliness of resident units, central bathing area, treatment and supply rooms, workrooms and linen closets; cleans bedpans, urinals, and commodes; washes basins and emesis basins; sanitizes bath tub/shower chair after each resident’s use; cleans floor if resident is incontinent; wipes spilled food and liquids
- Provides post-mortem care as instructed
Special Nursing Care Responsibilities
- Identifies and reports abnormal skin conditions
- Turns bedfast residents as instructed
- Provides Range of Motion exercises
- Assists with application of adaptive devices
- Provides daily indwelling catheter care
- Provides perineal care
Food Service Responsibilities
- Assures that residents are positioned appropriately for meals
- Assures that appropriate utensils are within reach of residents
- Serves food trays and assists with feeding as indicated
- Keeps residents’ water pitchers clean and filled with fresh ice and water (on each shift) and within easy reach of residents
- Delivers and documents all a.m., p.m., and bedtime snacks and supplements
- Provides hydration to all residents according to center policy
Documentation
- Accurately documents information in the clinical record as required by the resident’s condition
Admission, Transfer and Discharge Responsibilities
- Makes residents comfortable and orients as necessary
- Inventories and stores residents’ personal possessions as instructed
- Establishes and maintains good public relations with residents, families and other center guests and shows empathy toward residents
Care Plan Responsibilities
- Participates in care conference as indicated and follows individual resident care plans
Administrative Responsibilities
- Reports observations concerning structural, equipment, and furniture defects and malfunctioning to appropriate personnel
- Maintains confidentiality of necessary information
- Utilizes supplies and equipment properly and without waste
- Thinks and acts calmly and logically to meet unusual occurrences of the job without being thrown off stride
- Performs any miscellaneous work assignments as may be required
Certified Medication Aide Addendum
- Prepare and administer scheduled medications according to established routines.
- Monitor vitals to establish baselines to determine if medications can be administered safely.
- Evaluate resident needs for PRN medications in relation to bowel function, pain and other presenting problems. Administer PRN medications as directed by charge nurse. Record response to PRN medication in conjunction with charge nurse.
- Order medications etc. from pharmacies as directed by the charge nurse.
- Assist charge nurse in narcotic count.
- Monitor resident medications and medication sheets for accuracy.
- Stock med cart after each shift.
- Assist charge nurse with prescribed treatments, etc. per charge nurse discretion.
- Clean and organize medication rooms to ensure that all items are being properly stored in the appropriate locations on a weekly basis.
Education/ Experience/ Physical Demands
Education and Experience
- Successful completion of State Approved Nurse Aide Training and Competency Evaluation Program; Name must appear on State Registry as Nurse Aide in good standing; Ability to communicate with residents and nursing staff; Must have the ability to read and write.
- Prior experience as Nurse Aide preferred but not required
Certified Medication Aide Qualification
- Certified as a Medication Aide according to current state requirements.
Physical Demands
- Lifting 60 plus pounds maximum with frequent lifting, moving, and/or carrying of objects weighing up to 60 pounds. Exerting considerable force frequently, and/or intermediate force constantly to move objects and residents. Reaching, handling, hearing, visual near acuity, bending, stooping, and walking.

- Considerable exposure to disagreeable conditions including odors, infection, illness, and physical contact form disruptive residents

- Works with elderly residents, families, and other health care staff members
