Are you highly organised, adaptable, and ready to make a meaningful difference in community healthcare? We're looking for a motivated individual to join our Urgent Community Response (UCR) and Hospital at home VW team as a Clinical triage support. This is a key role in the service, you will be at the heart of coordinating urgent care for patients in their homes, helping to prevent hospital admissions and support recovery in the community.

Working across both the Mid and North Hampshire UCR teams, you'll support the smooth running of our fast-paced service by managing referrals, scheduling visits, communicating with families and professionals, and ensuring timely updates to clinical systems. You'll need excellent communication, IT, and problem-solving skills, and the confidence to work both independently and as part of a multidisciplinary team.

This role offers hybrid working, but regular weekly on-site presence in both localities is expected. You'll be supported by a friendly, dedicated team committed to delivering outstanding care and innovation.
