Job summary

A local charity organization is looking for Hospital Discharge Support Volunteers to assist patients recently discharged from the hospital. AGE UK IW - Could you be a Hospital Discharge Support Volunteer?

At times some patients who are deemed medically fit for discharge can spend extended periods in hospital. This may be because they live alone and there are concerns about them accessing the support they need once home. These patients do not need a full social care assessment but would benefit from a bit of extra support to make the process of getting home a bit easier. Alongside others you will play an integral role in providing reassurance and low level support to people living on the Isle of Wight who have recently been discharged.

This could include:

  • Telephoning or visiting clients in their own home
  • Preparing a light snack and/or hot drink
  • Collection of prescriptions
  • Completing a small food shop from a list, with cash
  • Escalating areas of concern.

Availability Details

This is a new project which will require volunteer support for approximately 1 -2 hours per week, dependent on clients needs. Each volunteer will be allocated to a designated client for approximately 4 - 6 weeks. This role will require availability on weekdays.
