Overview

Qualified Social Worker – Adults Hospital Discharge Team (HiCAF) in Runcorn (UK). Earn £35 per hour while making a vital impact as a Qualified Social Worker over the next 3 months in the Adults Hospital Discharge Team. This role is full-time and located in Runcorn, supporting Halton Council’s efforts at Whiston Hospital during the winter period. This is an exciting opportunity to showcase your skills and experience in facilitating efficient hospital discharges, helping those in need transition from hospital care to the community seamlessly.

Responsibilities
  • Work collaboratively with medical and social services teams to ensure safe and efficient hospital discharges
  • Manage a complex caseload, demonstrating the ability to work independently and think on your feet
  • Provide tailored support and intervention plans for individuals and families transitioning from hospital to community-based care
  • Liaise effectively with multidisciplinary teams to ensure seamless and person-centred care
  • Adapt quickly to a fast-paced environment and swiftly address the individual needs of patients
  • Mentor and guide less experienced staff members when needed
About You / Qualifications
  • Previous experience working within an Integrated Hospital Discharge Team is essential
  • Familiarity with the Eclipse system would be advantageous
  • Demonstrated ability to operate efficiently in a dynamic and high-pressure environment
  • A full UK driving licence and access to a vehicle is required
  • Strong interpersonal and communication skills to engage with diverse populations
  • A degree in Social Work or related discipline, with a commitment to continuous professional development
