A healthcare organization is seeking a senior community nurse for a fixed term position in the United Kingdom. The successful candidate will manage a team and deliver comprehensive community care across the Integrated Care system. Applicants should have extensive community nursing experience, excellent communication skills, and relevant qualifications. The position offers a salary range of £38,682 to £46,580 per annum, with a commitment to professional development and team collaboration.
This role is offered on a fixed term basis for 12 months to cover internal promotion. Secondment would be considered. In addition to being an expert in your field with extended competencies including vacuum dressings & syringe driver management you need excellent communication skills to conduct holistic assessments as part of a case‑management approach. You will be required to work within a dedicated referral centre, manage a defined caseload, delegate responsibilities to your Band 5 nursing team for continuity of care, and use electronic patient record systems such as Systm One (training provided). You will collaborate with Therapy colleagues, GP surgeries, Rapid Response and Complex Care at Home to deliver appropriate care. A valid driving licence and access to a vehicle for business use is essential.

Place based working requires you to become familiar with your own patch and liaise with local voluntary and third sector services, including the Community Wellbeing Service, to provide holistic, person‑centred care. You will be a senior community nurse or district nurse, supported by induction, clinical supervision and leadership guidance.
