The Purpose Of The Role

• To provide holistic and person-centred care and support community patients in an acute setting.
• The post holder is responsible for the assessment of care needs, the development, implementation and evaluation of programmes of care for each inpatient.
• To set the standards of care within the Ward and ensure that personal and professional practice contributes to overall excellence within the clinical area.
• To lead the day to day operation of the service within the Ward taking overall responsibility for the line management of staff and the provision of high quality services.
• To provide professional leadership and support to staff by bringing expertise in theory and practice to the ward.
• To be responsible for the day to day management of the sub-acute wards i.e., Health and Safety issues, acting within Trust policies and procedures.
• To maintain effective communication with various disciplines, relatives, patients and outside agencies within the clinical environment.

Main duties of the job

• Work directly with the ward clinical team including medical staff, nurses, allied health professionals and support staff to discuss, agree and record the patient's pre -admission and current level, identify and assess patients requiring support services or advice for discharge.
• To manage the service regularly taking charge of a shift whilst maintaining overall responsibility for the assessment of care needs and the development, implementation and evaluation of careprogrammes within the whole ward.
• To be a competent practitioner, leading innovation and demonstrating clinical expertise.
• To ensure that discharge planning commences at admission and ensures a smooth transition to home or other suitable environment.
• To manage staff within their ward/department including specific induction, recruitment and selection, deployment, training, performance management, sickness/ absence and appraisals.
• To ensure that nursing care satisfies the competence requirements of the NMC and Trust Policies.
• To manage the patient case load appropriately, to include delegation of work to team members, using resources effectively, demonstrating that the service is value for money.
• To ensure the development, production and implementation of the ward/department's objectives, in line with agreed service and financial objectives.
