A leading healthcare employer in Tampa is seeking a TOC Clinical Pharmacist. This role involves providing patient care, reducing hospital readmissions through follow-ups, and engaging in patient education. The ideal candidate should have a Bachelor's Degree in pharmacy, a valid Florida pharmacist license, and experience in patient care. BayCare is currently in search of our newest TOC Clinical Pharmacist who is passionate about providing outstanding customer service to our community.
The TOC Clinical Pharmacist shall support the mission of the hospital by aiming to reduce hospital readmissions through patient discharge follow-up. Perform clinical duties and engage in patient care to assist with medication safety, increase patient medication and disease state understanding, and encourage provider follow-up and continuity of care following hospital discharge.
Facility: St. Joseph's Hospital / Hybrid

Status: PRN - as needed, exempt
Shift: 9:00AM - 5:30PM
