Qualifications

  • Pharmacist license in Florida is required.
  • Preferred: ASHP (Pharmacist) certification.
  • Experience with PGY-1 or PGY-2 residency is preferred.

TOC Clinical Pharmacist

BayCare is currently in search of our newest TOC Clinical Pharmacist who is passionate about providing outstanding customer service to our community.

Clinical Pharmacist Responsibilities

The TOC Clinical Pharmacist shall support the mission of the hospital by aiming to reduce hospital readmissions through patient discharge follow-up. Perform clinical duties and engage in patient care to assist with medication safety, increase patient medication and disease state understanding, and encourage provider follow-up and continuity of care following hospital discharge.

Clinical Pharmacist position details:
  • Facility: St. Joseph's Hospital / Hybrid

  • Location: Tampa, FL

  • Status: PRN - as needed, exempt

  • Shift: 9:00AM - 5:30PM

  • Weekend Work: None

  • On Call: No
