What You'll Do:
  • Travel daily to assigned MN service areas to visit clients in their private homes
  • Assess communication and swallowing disorders, then develop and implement personalized plans focusing on dysphagia, speech intelligibility, and cognitive-communication needs
  • Provide interventions based on physician orders and educate caregivers in safe feeding techniques and communication strategies aligned with the care plan
  • Maintain detailed records per MDH and payer guidelines, ensuring accurate billing through proper CPT code usage and modifiers
  • Partner with the care team to review clinical progress, make adjustments to treatment plans, and ensure optimal patient outcomes
What You'll Need:
  • Active MN Speech Language Pathologist License
  • Master's degree in Speech Language Pathology
  • American Speech-Language-Hearing Association (ASHA) Certificate of Clinical Competence preferred
  • Experience with seniors, geriatric dysphagia, and home settings, preferred.
  • CPR certification
  • Reliable vehicle with valid license and insurance
