Overview

BayCare is seeking a Registered Nurse (RN) to join our Clinical Heart Failure Physician Office in Tampa. In this role, you’ll oversee advanced heart failure clinic operations while providing direct patient care throughout all phases of the referral process pre-, intra-, and post-care. You’ll work closely with physicians and an interdisciplinary team to reduce readmissions, optimize chronic disease management, and improve patient outcomes.

Responsibilities
  • Provide direct patient care and oversee clinic operations.

  • Train, schedule, and evaluate staff to ensure compliance with clinical and regulatory standards.

  • Collaborate with physicians, social workers, and interdisciplinary teams to resolve barriers to care.

  • Escalate complex clinical, social, or environmental issues to the appropriate channels.

  • Support patients with high acuity needs and identify alternative care pathways when necessary.

  • Perform other duties as assigned.

Qualifications
Education
  • Required: Associate’s/Technical degree in Nursing

  • Preferred: Bachelor’s in Nursing

Licensure & Experience
  • Required: RN license – Florida Dept of Health

  • Required: 1-year Licensed RN experience

  • Highly Preferred: 3+ years in cardiology, heart failure, or related specialty

  • Highly Preferred: AAHFN certification

Skills
