Adult Foster Care Nursing Program

Our Adult Foster Care (AFC) Program is a Medicaid‑funded initiative that enables individuals with disabilities to live with caring providers in a safe and nurturing environment. Nurse staff monitor health status, provide case management, and serve as a resource for all program staff and home.

Responsibilities

• Coordinate medical information for admission and provide a comprehensive nursing assessment for the intake process when required by individual medical condition(s).
• Provide participant‑specific medical training for home care providers and nursing case training as needed.
• Monitor medical care to ensure that consumers receive quality care, treatment, and respect in the community.
• Maintain individual confidential files and monitor placements to ensure compliance with all applicable regulations.

Qualifications

• Current licensure to practice as a Registered Nurse by the Commonwealth of Massachusetts Board of Registration of Nursing.
• Associate’s or Bachelor’s in a relevant field is preferred.
• A minimum of two years prior experience with elderly or individuals with disabilities.
• Proven ability to utilize electronic health record systems to monitor compliance (currently iCentrix and HCSIS).
