Physical Therapist | Home Visits – Lakeshore | Part-Time

Atrio Home Care, a trusted partnership between Holland Home and Resthaven, is seeking a compassionate and motivated Physical Therapist to join our Holland/Lakeshore team, serving clients in Ottawa and Allegan Counties.

This part-time role offers the perfect blend of autonomy, flexibility, and purpose—ideal for someone who values personalized care and work-life balance.

What You’ll Do
  • Provide one-on-one in-home physical therapy services
  • Conduct evaluations and develop individualized treatment plans
  • Collaborate with physicians and interdisciplinary team members
  • Help clients regain mobility, strength, and independence
  • Document progress and support discharge planning
  • Educate clients, caregivers and families on therapy goals and techniques
Schedule & Location
  • Territory: Ottawa & Allegan County
  • Days: 3 days/week (approximately 24 hours) – flexible scheduling
  • Shift: Day shift (8 AM – 5 PM)
  • Weekend Rotation: Every 4–5 weekends, no on-call
  • Holidays: 1–2 per year
Compensation & Perks
  • Pay-for-activity model with weekend/holiday differentials
  • Estimated earnings:
    • New grads: ~$85K
    • Experienced PTs: $95K–$110K
    • Bonuses: Productivity, case management, quality care, and client satisfaction
  • Orientation: 6–8 weeks with 1:1 field training ($320/day)
  • Mileage reimbursement and additional pay for meetings/in-services

What You Bring
  • Doctorate in Physical Therapy (required)
  • Michigan PT license (required)
