Responsibilities
  • Conduct telephonic outreach for scheduling and health maintenance.
  • Review patient records and reports for care gaps.
  • Task physicians and staff for care gaps.
  • Request records to fulfill care gaps.
  • Contribute to collaboration for best practice with employed and embedded staff.
  • Mailings as required for campaigns or wellness visits.
Qualifications
