On-Call/Triage Hospice Nurse (Per Diem) at Hospice of Southern Maine
Hospice of Southern Maine is seeking a per diem On-Call/Triage Nurse to work in our Home Program. The On-Call/Triage Nurse is the first line contact for after-hours agency communications and patient care needs to ensure access to 24/7 nursing services per agency policy. The nurse handles inbound after-hours calls, assesses patient and family needs, provides education about interventions, and coordinates or performs required care and follow-up in person, electronically, and by telephone in homes, facilities, and hospitals.
The On-Call/Triage Nurse conducts symptom management and emergent nursing care; continuous home hospice care visits; referral management, admissions, and transfers to the Gosnell Memorial Hospice House; Time of Death, post-mortem, and RN bereavement visits. The Triage Nurse also supports RN Case Managers by following up on physician orders, prescription refills, and test results; provides targeted teaching to home patient caregivers; coordinates care with the Hospice Interdisciplinary Team; and makes proactive outreach calls to patients with rapidly changing conditions. All services are provided pursuant to a physician’s orders and consistent with the patient’s plan of care.
The on-call/triage hours are 4pm-8:30am and incumbents are paid their hourly rate for the entire on-call shift, regardless of the number of calls or visits. On-call shift days vary based on needs. Flexibility to cover various days of the week is required. The ideal candidate would also have availability to cover day shift per diem visit RN hours if there are no on-call/triage shifts needed.
Qualifications include a graduate of an NLN-accredited nursing program; BSN or CHPN preferred. Minimum of two to three years of RN experience required. Home care, hospice, and/or oncology nursing experience preferred. Pain management, symptom control, crisis intervention, and family support experience required. Registered Nurse licensure by the State of Maine; valid driver’s license with an automobile that is insured and in good working order. BLS certification or the ability to obtain certification within the first 90 days of employment; certification must be kept active during employment.
