Job description

Home Health: Physical Therapist (PT) Case Manager (North Central)

$5000 Sign-On Bonus

Pay: $91,000 - $100,000 per year + mileage reimbursement ($0.70 per mile)

Schedule

Monday to Friday, 8:00am-5:00pm (no weekend/holiday on‑call; rotating weekend every 10th weekend, 2 holidays per year)

Responsibilities

• Provide physical therapy services per physician’s written plan of care and state PT Practice Act.
• Assess therapeutic status, home environment, muscle strength, mobility, gait, ROM.
• Direct PT treatment and instruct clients/caregivers in appliance use.
• Determine priority PT needs, report to physician, adjust plan as needed.
• Initiate PT program, instruct staff/family and set goals for case conferences.
• Refer for other home health services and submit clinical progress summaries.
• Participate in discharge planning and peer consultation.
• Supervise PT assistants and home health aides per policy and state regulation.
• Maintain client confidentiality, privacy and report maltreatment.
• Follow all emergency and response procedures.
• Identify home safety issues and notify RN Case Manager or Clinical Manager.

Qualifications

• Minimum 2 years of clinical PT experience; Community/Home Health experience preferred.
• Graduate of an APTA-accredited PT curriculum.
• Current valid PT license in Minnesota.
• COVID vaccine verification or approved exemption.
• Master’s degree required.
