Congestive Heart Failure Clinic - Nurse Practitioner (APRN) or Physician Assistant

Location: 85 Seymour Street - HHMOB (10117).

Hartford HealthCare Heart & Vascular Institute brings together cardiac care specialists to provide integrated care with the latest technology. The Center for Advanced Heart Failure & Pulmonary Vascular Disease supports advanced treatments such as ECMO, heart pumps, and heart transplant care. The Advanced Practitioner role supports inpatient and outpatient heart failure management within a multidisciplinary team.

Responsibilities
  • The Advanced Practitioner will work at Hartford Hospital with The Center for Advanced Heart Failure team, providing comprehensive inpatient and outpatient care in the heart failure management program.
  • Collaborate with a multidisciplinary group of providers including advanced practice providers, physicians, dietitians, nurses, and medical/administrative staff.
  • Contribute to the outpatient heart failure management program, including a Bridge Infusion Program, HF Clinic, PA Sensor program, and VAD Program to manage heart failure-related volume overload and reduce hospital admissions.
  • Demonstrate a passion for cardiology and adaptability in a rapidly growing practice.
Schedule
  • Four 10-hour shifts per week
  • Some weekend coverage required (rotating basis)
  • No overnight responsibilities

Qualifications
  • Graduate of an accredited Physician Assistant / Nurse Practitioner Master’s program
  • Licensed, or eligible to be licensed, as a Physician Assistant / APRN in the state of Connecticut
  • Certification or eligible to be certified as a Physician Assistant or National Board Certification as an APRN
  • Minimum 1–2 years of experience in cardiology, critical care, or advanced HF preferred (new grads with strong clinical backgrounds may be considered)
  • LVAD experience preferred but not required
