Overview

Case Manager, High Support Housing, Dublin. Location: Dublin. Contract: Full Time, 2 Year Fixed-Term Contract.

Purpose of this role: to work as part of the core DOSH (Dublin Off-Site Housing) team supporting households with complex needs, with a focus on individuals who have experienced long-term homelessness.

What You Will Do
  • Deliver services in line with Focus Ireland’s standards of best practice.
  • Operate within the objectives and policies of the Dublin Offsite Housing team.
  • Collaborate with the Focus Housing Association property management team.
  • Provide a safe, welcoming environment for customers, offering basic physical care and support while respecting individual choice and autonomy.
  • Build trusting relationships with individuals who are street homeless or in temporary accommodation, supporting their transition to a more stable lifestyle.
  • Manage a caseload of 10–12 customers, conducting assessments, referrals, and support planning as the lead agent using the Care and Case Management system and other relevant tools.
  • Support clients in accessing housing, welfare rights, entitlements, and move-on accommodation.
  • Assist customers in creating and sustaining a home, primarily through face-to-face, in-home support aligned with tenancy settlement indicators.
  • Accompany customers to appointments (e.g., doctors, counsellors, court).
  • Collaborate within a multi-disciplinary team, coordinating wraparound supports with internal and external agencies.
  • Support customers in developing homemaking skills and maintaining positive relationships with their housing officer, focusing on rent payment, home management, repairs, and neighbourliness.
  • Help customers integrate into their local community and access clinical, medical, and other support services.
  • Encourage life skill development and support access to education, training, and employment opportunities.
  • Maintain accurate casework records, daily logs, and assessments using Focus Ireland’s database and PASS.
  • Attend internal and external meetings, handovers, and training sessions as required; participate in regular supervision with your assigned supervisor.
  • Supervise students or volunteers as requested and implement existing policies while contributing to the development of new ones.
  • Undertake other duties as reasonably assigned by the Service Manager or Team Leader.
  • Work a 5 day week and be flexible regarding working hours; some weekends may be required.
  • Remain vigilant to health, safety, and welfare risks in the workplace and report concerns accordingly.
What You Will Need
  • Degree-level qualification in social care, youth work, or a related discipline.
  • Experience working with individuals who are or have been homeless.
  • Experience supporting individuals with dual diagnoses.
  • Strong team collaboration skills.
  • Proficient report writing and IT skills.
  • Experience liaising with external agencies.
  • Excellent communication skills.
  • Understanding of homelessness and related issues.
  • Ability to plan and implement support programmes.
  • Flexibility in approach.
  • Full driver’s license (desirable).
