As a Wellbeing Support Worker, your role involves carrying out triage calls to assess the initial level of need of people referred to the service. You will offer 1-1 support to assess individual needs and co-produce a support plan with the service user. You will assist and aid service users to maximise their independence as part of a structured programme of care and support.
