• At least one year of post-CCT experience in primary or urgent care.
• Registered with the GMC and on the GP Specialty Register.
• Inclusion on the NHS Performers List is required.

Responsibilities

• Undertake both planned and urgent primary care as required.
• Provide patient-focused, evidence-based practice to a high standard.
• Support the MDT to ensure proper care for long-term health conditions.
